About ProCare Hospice

handsProCare Hospice uses a family-centered approach to end of life care for the patient and family. Our goal is to provide care for the patient and their families in an atmosphere in which remaining life can be completed with maximum independence, comfort, compassion and dignity.

We use an interdisciplinary team approach. The team consists of the patient and family, Hospice Medical Director, Primary Physician, the Hospice Nurses, Social Services, Spiritual Care and the Certified Hospice Health Aides. This team approach encourages the patient, family members, and the professional team to exchange knowledge and communication on the treatment benefits and preferences.

As a unit of care, the team, including the patient and family, anticipate problems and address them, rather than waiting for issues to manifest. The interdisciplinary team will coordinate and manage the care across all settings including the transition to hospice is necessary. The patient/family as a unit will work closely with the hospice staff. Together, they will actively participate in developing a plan of care that attains the outcomes that reflect the personal wishes, values and goals best suited for the particular needs of the patient.

The Hospice Physician directs the patient care. The patient may still be seen by his or her primary physician but treatments are coordinated through Hospice. Hospice Team Members include:

  • Physician
  • Nurses
  • Social Service
  • Spiritual Care
  • Home Health Aide and Volunteers

Additional services may include:

  • Music Therapist
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Respiratory Therapy
  • Pet Therapy
